An integrative approach to substance use, mental health & recovery. For individuals seeking intensive outpatient treatment for substance use, depression, anxiety, trauma, and other co-occurring disorders while continuing to live at home.
An intake assessment and screening are required before placement in the IOP.

9 hours per week
10 participants maximum
Program Investment
$3,750 total
$150 — Intake Assessment
$3,600 — Six-Week IOP
($200/day × 18 program days)
Insurance: Out-of-network with insurance. Reimbursement support is available upon request.
HOW IT WORKS
Your First Step Is an Intake
1. Schedule your intake
Use the Schedule Your Intake button to book an appointment with Sage.
2. Complete your intake assessment
The $150 intake assessment is required before placement in the program.
3. Screening & placement
Sage will review your needs and determine whether the IOP is an appropriate level of care.
4. Begin the program
If approved, you’ll be placed in the six-week IOP beginning November 2
Scheduling an intake does not guarantee placement in the IOP. All participants must complete the screening process before joining the group. During your intake, Sage will gather information about your substance use history, previous treatment, mental health needs, and goals for treatment.
